2. List of TV equipment for the release of their equipment from the Bureau of Customs.

(Must be ATA Carnet registered)

3. Consolidated crew list. (Use excel form attached)
4. 1pc 2×2 colored picture, white background, taken within six (6) months of submitting your application and reflect your current appearance for each of the production crew members.
5. Photocopy of their passports. (pages 1 and 2)
6. 3-5 bylined photo/articles published not later than 3 months.
7. Properly filled out IPC application form.
