2. List of TV equipment for releasing their equipment from the Bureau of Customs. (Must be ATA Carnet registered)

3. Consolidated crew list.

4. 1pc 2×2 colored picture, white background, taken within six (6) months of submitting your application and reflecting your current appearance for each production crew member.

5. Photocopy of their passports. (pages 1 and 2)

6. 3-5 bylined photos/articles published within 3 months.

7. Properly fill out the IPC application form.

8. Confirmed Flight Details (Optional).
