ACCREDITATION REQUIREMENTS

1. Online Accreditation Form properly filled up.
2. Scan Copy of Letter of request from respective agencies
3. Digital copy, 2’x2” colored pictures with white background (Pls. follow the format FIRSTNAME_LASTNAME.jpg)
4. Scan Copy of contract of employment (for local hire)
5. Scan copy of passport (pages 1& 2)
6. Scan Copy of Letter / Endorsement from the country embassy in Philippines
7. Health Declaration Form properly filled up.

ADDITIONAL REQUIREMENTS FOR NEW APPLICANT
AND FREELANCER
